Sudden Departure Shakes U.S. Health Leadership
The head of the U.S. Food and Drug Administration, Marty Makary, has resigned following months of political pressure, internal disagreements, and growing controversy surrounding the agency’s direction under President Donald Trump’s administration.
The resignation was confirmed Tuesday by multiple U.S. media outlets, marking another major shake-up inside America’s public health system. Makary had served as FDA commissioner since March 2025 and became one of the administration’s most recognizable health officials.
Sources familiar with the matter said tensions between Makary and senior White House officials had intensified in recent weeks over several high-profile policy decisions involving vaccines, vaping products, and pharmaceutical regulation.
Disputes Over Vaping and Drug Policies
One of the biggest conflicts reportedly centered on the FDA’s handling of flavored electronic cigarettes. Makary had initially opposed approving certain fruit-flavored vaping products, citing concerns about youth addiction and rising teenage vaping rates.
However, pressure from industry groups and political allies reportedly pushed the agency toward a more flexible position, creating friction inside the administration.
The FDA also faced mounting controversy over its review of abortion medication policies, particularly involving mifepristone. Conservative groups accused the agency of moving too slowly on tighter restrictions, while public health advocates warned against political interference in scientific decisions.
The disagreements highlighted the increasingly difficult position of the FDA as it navigated pressure from both political parties, health activists, and major pharmaceutical companies.
Makary Became a High-Profile Figure During Trump’s Second Term
Makary, a surgeon and former professor at Johns Hopkins University, was widely known before joining the FDA for his outspoken criticism of COVID-19 lockdowns, vaccine mandates and pandemic-era restrictions.
After being appointed by Trump, he became closely linked to the administration’s “Make America Healthy Again” campaign, which aimed to reshape federal health agencies and reduce what officials described as excessive government intervention in healthcare.
Supporters praised Makary for challenging established public health institutions and advocating for greater transparency inside federal agencies. Critics, however, accused him of politicizing scientific decision-making and undermining confidence in vaccine oversight.
Concerns Grow Over FDA Stability
Makary’s departure comes during a period of significant instability at the FDA and across the broader U.S. health system.
Several senior officials responsible for vaccines, drug approvals and public health oversight have left federal agencies over the past year. Analysts say the repeated leadership changes could disrupt major pharmaceutical reviews, medical research coordination and regulatory decisions affecting both the United States and global healthcare markets.
Healthcare experts also warned that uncertainty at the FDA may create delays in approvals for new medicines and medical technologies at a time when the agency is already facing intense scrutiny.
White House Expected to Name Interim Leadership
According to U.S. reports, Makary’s deputy, Kyle Diamantas, is expected to serve as acting commissioner until the White House appoints a permanent replacement.
President Trump briefly addressed the resignation while departing for an overseas summit, reportedly describing Makary as “a capable doctor” while acknowledging disagreements over policy direction inside the agency.
The White House has not yet announced a timeline for selecting a new FDA commissioner.
FDA’s Global Influence Remains Significant
The FDA is one of the world’s most influential health regulators, overseeing vaccines, medicines, food safety, medical devices and tobacco products in the United States.
Its decisions often shape healthcare standards and pharmaceutical markets worldwide, meaning leadership changes inside the agency are closely watched by governments, investors and healthcare companies around the globe.
Makary has not yet released a detailed public statement explaining the reasons behind his resignation.
